When working with the Numbers application we recommend following these steps here:
1: Open the "Sample Outlooks CSV" in Numbers
2: When the table opens it pulls up, click the Adjust Settings options
3: Click "Delimited" at the top of that box and make sure it's set to comma and not space or otherwise. Click Update Table.
4: Paste/fill in fields. Go to File > Export > CSV.
5: Ensure the "Include table names" box is UNCHECKED. Hit Next
6: Re-title the document as needed.
7: Import your file from under Contacts >> Import/update contacts >> From Outlook CSV file
